Wound Care Program
Esperta Health delivers a complete wound care continuum through two integrated models: in-home specialist care for wounds that need hands-on intervention, and virtual wound care for wounds that can be managed remotely, with seamless transitions between the two as clinical needs change.

Wound Care Expertise
Esperta Health delivers advanced wound care directly in the patient’s home through a team of wound-certified clinicians supervised by ABWM-boarded wound physicians. Clinical services include comprehensive wound assessment, wound cleansing, advanced dressing application, local anesthesia, sharp and non-sharp debridement, and negative pressure wound therapy. Dressing supplies are delivered directly to the patient’s home.
Specialized Physicians and Nurses
Every clinician is certified in wound care. Physicians are boarded by the American Board of Wound Management, and nurses are certified through the Wound, Ostomy and Continence Nursing Certification Board, the National Alliance of Wound Care and Ostomy, or the Esperta Health wound certification program.
Every Initial Visit Is Led by a Nurse Practitioner
Each initial patient visit is conducted by a nurse practitioner who performs the clinical intake, establishes the diagnosis, and sets the plan of care. This is a meaningful distinction from most home-based programs, where initial visits are routinely performed by RNs under standing orders.
Wound Evaluation System
At each visit, our proprietary Wound Evaluation System collects and integrates five clinical inputs, captured consistently across every clinician and every visit to eliminate the variability common in traditional wound care.

Responsive Treatment Plans
The Wound Evaluation System feeds directly into the Responsive Treatment Plan. A proprietary algorithm analyzes the patient’s history and exam findings against our embedded guidelines to generate up to 15,000 individualized treatment plans. The plan is regenerated at every visit based on updated clinical data and the patient’s response to treatment. The treating clinician reviews, edits as clinical judgment requires, and signs off before any orders are executed.
Each plan has three key outputs:

Wound Orders
An order set with wound-specific supplies is automatically developed for the in-home clinician to treat the wound.

Healing Instructions
Patient-facing guidance covering wound care, wound etiology, recommended lifestyle modifications, and nutrition.

Tailored Dressings and Supplies
Supplies are ordered straight from our platform and delivered to the home, improving standardization and efficiency.
Wound Prevention Program
Upon wound closure, members enter a structured 12-week wound prevention program.
Clinicians monitor members remotely through virtual technology and intervene at any indication of recurrence.
This protocol is a primary driver of our wound recurrence rate of less than 1%.

Innovative Wound Technology
Our wound platform gives our specialists the accuracy and flexibility needed to capture all the pertinent information about the patient’s wound.
Once the patient’s data is entered into the wound evaluation system, Esperta Health’s proprietary algorithm analyzes over 15,000 Responsive Care Plans. In-home care specialists treat patients with the recommended plan of care.
